New Patient Forms
After making your appointment for your initial consultation with Smile Experience Orthodontics, you will receive an email from us with new patient forms to complete. It will save you time if you have an opportunity to fill them out before your appointment. However, if you do not get a chance to complete these forms prior to your appointment, you can complete them here at the office when you arrive. It’s a good idea to allow 15 to 20 minutes before your appointment to fill out these forms in-office and provide any other information requested. Adults should bring a copy of their dental health insurance so that we can confirm your orthodontic benefits.
You can expect to leave our office after your consultation with a customized treatment plan with all of your options, including the cost of treatment and your orthodontic insurance benefits.
Financial Information

Dr. Rosenberg and his team will discuss with you the cost of your treatment and be happy to help you decide what financial option is best for you. Accepted Insurance
Dr. Rosenberg and his team will work with your insurance provider to maximize your orthodontic benefits. Just provide us with your insurance information, and we will take care of the rest—including calling your provider to verify all your benefits, filing all insurance claims, and making sure all insurance payments are received and credited to your account.
